Smithtown Booster Club Scholarships

If you would like to be considered for one of the Smithtown Booster Club Scholarships please CLICK HERE to download the application.  REQUIREMENTS FOR A COMPLETE APPLICATION: 2 page application and Letter of recommendation from your coach.

All completed applications must be mailed or dropped off at:

Smithtown Athletic Office 26 New York Avenue, Unit 1 Smithtown, NY 11787

ALL Applications must be Submitted or Postmarked by June 11th, 2021 Any application received or postmarked after this date will not be accepted.

 In order to be eligible to receive the Booster Club $1,000 scholarship, membership in the Booster Club is required. Membership has to be done by June 10. Non members are still eligible for a $250 scholarship if selected. Click Here To Become a Smithtown Booster Club Member
